The North Muskoka Nurse Practitioner-Led Clinic [NPLC] is a non-profit health care agency funded by the Ministry of Health [MOH]. Our vision is to “Improve Health and Wellness Together”.  The North Muskoka NPLC is currently looking for;

Social Worker (MSW) – Permanent Full Time

As a member of an interdisciplinary team the social worker will take a lead role in providing clinical assessment, treatment and educational services to clients of the NMNPLC.  The social worker’s role will also be to contribute to the ongoing development of structure, policy and capacity meeting the goals and needs of individual patients.

Qualifications:

  • Registration with the Ontario College of Social Workers and Social Service Workers
  • 3-5 years relevant work experience
  • Excellent inter-personal, communication and decision-making clinical skills
  • Demonstrated ability to teach, counsel and develop written educational materials
  • Demonstrated ability to work independently and as a member of a multi-disciplinary health services team

Nurse Practitioner – Temporary Contract

0.8 FTE

Covering a leave of absence, the Nurse Practitioner will be a highly valued and respected team member working to his or her full scope of practice within a supportive, energetic and positive environment.  The NP will work with a variety of clients, assisting individuals and families to achieve and maintain their optimal level of health.

Responsibilities:

  • Provides ongoing comprehensive primary health care to clients of all ages within the Nurse Practitioner scope of practice.
  • Incorporates and strengthens collaborative and inter-professional work;
  • Maintains complete and accurate client records;

Qualifications:

  • Certificate as a Primary Care Nurse Practitioner from a recognized institution;
  • Current registration in the Extended Class with the College of Nurses of Ontario;
  • Experience in a primary care setting, or in combination of community and hospital or public health settings;
  • Demonstrated ability to work in an inter-professional team environment;
  • Excellent communication, interpersonal and time management skills;
