Registered Social Worker – Permanent Full Time

 Qualifications:

  • Registration with the Ontario College of Social Workers and Social Service Workers
  • 3-5 years relevant work experience
  • Excellent inter-personal, communication and decision-making clinical skills
  • Demonstrated ability to teach, counsel and develop written educational materials
  • Demonstrated ability to work independently and as a member of a multi-disciplinary health services team

Job Description – Social Worker

OVERVIEW:

As part of an interdisciplinary primary health care team, the social worker will provide assistance for clients and their families coping with challenging situations in order to maximize their health and wellness, while ensuring that high quality patient care is delivered, consistent with the mission and values of the North Muskoka NPLC. The Social Worker will design and implement an in-house mental health program, offer ongoing counseling, complete and follow-up on referrals to outside organizations that provide social or financial assistance, legal aid, housing, or in- home supports, perform house calls and community outreach visits as required; consult and collaborate with inter professional team members.

ROLES AND RESPONSIBILITIES:

MENTAL HEALTH COUNSELING:

  • Provide ongoing mental health counseling to clients and their families across the lifespan including but not limited to; depression, anxiety, domestic violence, addictions, trauma, relationship stress, couples and family counselling, chronic disease, financial stress, geriatric health, caregiver support, palliative care, advance care planning.
  • Be able to link with community resources and supports to enhance in home support that address the social determinants of health and promote wellness.

CHRONIC CONDITION MANAGEMENT:

  • Participates in the development and evaluation of individual and group education sessions aimed at maximizing health for those with chronic conditions.
  • Collaborates with interdisciplinary care team to ensure appropriate management and follow-up as required.

EVIDENCE-BASED PRACTICE

  • Practice evidence based care and therapy which may include but is not limited to; CBT, DBT, MCBT, Narrative Therapy, MI, EFT or PACT therapy for couples.
  • Keep current on the latest research in the field/ areas of practice.
  • Participate in the education of students at the clinic upon request.

COMMUNITY DEVELOPMENT

  • Participate in community advocacy initiatives such as Bridges Out of Poverty Advisory Board, Elder Abuse Prevention Muskoka, Seniors Planning Table, Master Aging Plan, Living Wage, DART committee, OASW Healthcare Advisory Committee and other areas of interest.
  • Collaborate with agencies/services in the community, and family members, wherever appropriate, for the purpose of consolidating and coordinating client treatment.
  • Engage with other local Mental Health Therapists and/or groups to help maintain collaborative relationships and to help foster a supportive learning environment.
  • Promote the NMNPLC in the community at large.

PRE-REQUISITES

  • Masters level education in an appropriate field preferred.
  • Registration in good standing with the Ontario College of Social Workers and Social Service workers.
  • Minimum 3 years related or applicable experience in a community or primary health care setting
  • Member of Ontario Association of Social Workers preferred.
  • Satisfactory Criminal Record Check
